What information do employees need to give consultants for the Employee Assistance Program — Work/Life Balance services?

The consultant will ask for your name, phone number, email address, city and the nature of the call. Email is requested to make referrals to local resources and provide any requested information, and it also helps with subsequent calls. All information provided is confidential.

The Employee Assistance Program — Work/Life Balance services provided by HealthAdvocate — are available with Unum’s long-term disability insurance purchased through GuideStone®.

Terms and availability of service are subject to change. Service provider does not provide legal advice; please consult your attorney for guidance. Services are not valid after Unum coverage is terminated.
