If you are enrolled in coverage, log into your MyGuideStone® account, hover over the “Insurance” tab, and select “Plan Documents”. Here you will find all the plan booklets related to your specific coverage.
You may also access all plan booklets here.
Administrators may view the Summaries of Benefits for their group's specific health plans in the Employer Access Program "Document Center".
