What do I need to do if my employee would like to add life coverage for themselves and/or a dependent in Group Plans?

If your group offers life coverage in Group Plans, your employee may be enrolled in your group's guaranteed standard issue amount of life coverage without underwriting if requested within 31 days of his/her eligibility date. After this date, your employee must complete an Evidence of Good Health Application to begin the application process for coverage.

Please note each group may have different coverage amount limits.

Once your employee has completed the application, it must be submitted to your designated Operations team. The team will process and submit the application to our vendor for underwriting, which can take up to 8 weeks. While instructions are included on the application, please contact your designated Operations team with any questions.
