What do I need to do if my employee would like to add life coverage for themselves and/or a dependent in Group Plans?

If your group offers life coverage in Group Plans, your employee must complete an Evidence of Good Health Application in order to begin the application process for coverage.

Please note each group may have different coverage amount limits.

While instructions are included on the form, please contact your designated Operations team with any questions. Once completed, submit the form to your Operations team.
