Why does GuideStone have a PHI Authorization form? What is it for?

To ensure GuideStone is protecting its participants’ protected health information (PHI), individuals over the age of 17 must complete GuideStone’s PHI Authorization form to authorize someone other than themselves to discuss their personal PHI with a GuideStone representative. The following cases are examples of when a PHI form is needed: a participant’s spouse needing to speak on behalf of the primary participant regarding the primary participant’s claim information, a parent wanting to speak on behalf of their 19-year-old daughter regarding the daughter’s claim information, etc.
