It is the member’s responsibility to obtain the prior authorization. The member or ordering physician calls a dedicated number, 1-800-424-4922, which is on the back of your Highmark BCBS ID card. Information regarding the service will be reviewed. If approved, the authorization is valid for 60 days from the date of decision. An appeal process is available if the prior authorization is denied.
