Does my Consumer-Driven Health Plan have co-pays?

No. Because your plan is a HSA-qualified High Deductible Health Plan (QHDHP), it does not have co-pays. Until your in-network deductible is met, you will pay the full cost after the plan’s discounts of the eligible covered medical services and prescription drugs. Once your in-network deductible is satisfied, you will pay applicable co-insurance until your maximum out-of-pocket is satisfied. Out-of-network services apply to separate deductible and co-insurance levels.

Co-insurance is the percentage of eligible expenses you and the plan share for covered services or supplies not covered by a flat co-pay. The exact co-insurance depends on the plan provisions. Your co-insurance will be the amount of Covered Services or supplies which must be paid by you.
