Class I: Preventative and Diagnostic Care for dental insurance includes:
- Routine oral examinations — two times per calendar year
- Routine dental cleanings — two times per calendar year
- Bitewing X-rays — once every 12 months
- Fluoride treatments for children under age 14 — one treatment every 12 months
Class II: Basic Restorative Care for dental insurance include:
- Panoramic X-ray — once every 60 months
- New fillings/replacement fillings — once every 24 months per filling
- Simple extractions
Class III: Major Restorative Care for dental insurance include:
- Endodontic treatment (root canal)
- Dentures
- Crowns
- Implants
- Prosthesis over implant
Class IV:
