What does co-insurance maximum mean?

The co-insurance maximum is a specified dollar amount of Out-of-Network Eligible Expenses Incurred by a Covered Person for Covered Services and Supplies in a Benefit Period, after which the level of benefits is increased as specified in the Schedule of Benefits. Such expense does not include the amount of charges in excess of the Provider’s Reasonable Charge and penalty amounts incurred by the Covered Person under this Plan. This maximum excludes deductible and co-payments.
